Workflow principle
CliniTrack connects to the patient-facing Track+ experience while keeping the care-team narrative focused. The goal is not more data for its own sake. It is better preparation, more timely support, and fewer context gaps.
Consent architecture
The Track+ to CliniTrack linking model is being designed around two-sided pairing, explicit patient authorization, limited data scopes, and a clear boundary between an active shared workspace and records a clinic must independently retain.
A patient and provider exchange short-lived codes, and neither code alone transfers information or activates a clinical connection.
Patients review which categories are requested and separately choose whether existing history and future records may be shared.
Authorized clinic users receive only the approved patient-created information, while originals remain attributable and protected from overwrite.
The design calls for visible access history, revocation controls, export, and a new consent grant before any disconnected relationship is reactivated.
